Transfering Patient to Operating Room

[Narrator]

One day Hellen goes to school. She is 25 years old. She collage in Diponegoro University. After her school finished, she bought cimol and seblak so spice. Suddenly she felt pain on her stomach. So, she come back to her home quickly. In her home, she told with her mother that she got pain on her stomach. Hellen’s mother worried about it. So, her mother suggest Hellen to go to the hospital. Until the hospital, Hellen entered emergency room. She accompany by her mother. In emergency room.....

Patient : “Mom, I feel pain Mom.”

Mother : “Be patient baby, nurse will help you.”

Patient : “Awwww so pain,Mom.” (crying)

Suddenly nurse come in room

Mother : “Nurse please help my child.”

Nurse 1: “Yes mrs, I will check her and assessment before.”

Nurse Kurniati do pain assessment to hellen. The result of PQRST assessment is hellen says that she felt pain because she ate cimol and seblak so spice, she felt pain with  scale 5 on right below of her stomach. She felt pain continued. Gbvyh1uAfter did assessment nurse suggest Hellen to take a rest , to decrease her pain. Next nurse out from room and meet a doctor to give the result of assessment.

Nurse   1: “Ok I have done to check your condition, and you can take a rest so your pain can

decrease.”

Patient : “Ok, Nurse so pain.”

Nurse   1: “I know that, i will give assessment result to doctor, so a doctor can get medical

diagnoses.”

Patient : “Ok, Nurse.”

A nurse out from emergency room and meet a doctor.

Nurse   1: “Doctor, this is the assessment result of Hellen.”

Doctor : “Oke nurse, thank you. From the result I will check up and rontgen to her stomach.”

Nurse 1: “Yes, Doctor.”

A doctor check up and rontgen to Hellen’s stomach. The result is she got appendicitis. Then the doctor told to Hellen’s mother about the condition.

Doctor : “Good morning Mom. Please, sit down.”

Mother : “Thank you, Doctor. How the condition of my child?”

Doctor : “Well. So, after I check up your child. She got appendicitis.”

Mother : “What is that?”

Doctor : “Appendicitis is inflammation of appendix. Appendix is the part of colon.”

Mother : “So, what should we do to resolve the appendicitis?”

Doctor : “I think we must to cut the appendix, because the inflammation of the appendix can

make more infection on her stomach.”

Mother : “Do you have any way to resolve it?”

Doctor : “I think it is the better way, Mrs.”

Mother : “Then, what the risk of it?”

Doctor : “I think the risk is blooding and pain on her scar. But, We will give medicine to reduce

the pain. Do you agree Mrs?”

Mother : “Emm.. Okay, I agree. Please help my child.”

Doctor : “Sure. We will do the best for your child.”

Mother : “Thank you.”

Doctor : “Yes, Mrs.”

Hellen’s mother back to the Hellen’s bed. A nurse come to Hellen’s bed to give information about her condition.

Nurse 2: “Hello hellen, good morning.”

Patient : “Morning, Nurse.”

Nurse   2: “Let me to introduce my self. My name is nurse Kurniati. I’m a nurse on duty in this morning shift.”

Patient : “Yes, Nurse Kurniati.”

Nurse   2: “Hellen, based on result of your checkup, you got appendicitis.”

Patient : “What is it?”

Nurse   : “Appendicitis is inflammation of appendix. Appendix is the part of colon. Doctor

suggest to cut your appendix, because it can make more infection on your stomach.”

Patient : “Then, what the risk of it to me, Nurse?”

Nurse   2: “I think the risk is blooding and pain on her scar. But, We will give medicine to reduce

the pain.”

Mother : “Yes, that right. I hope you can receive it. It is better to you.”

Nurse   2: “The operation will be held tomorrow at 2 p.m. So, you don’t eat and drink anything

until your time for operation finished.”

Patient : “Ok, Nurse.”

Nurse   2: “Now I need your signature to accept this intervention.”

Patient : “Yes, Nurse.” (signature inform consent)

Nurse  2: “Thank you. Now I have to go back to nurse station. If you need any help, you can call

me at nurse station or you can press button on your side bed.”

Patient : “Ok, Nurse.”

The time to operation is coming. And doctor visit Hellen’s bed to inform if her operation will be held.

Doctor : “Excuse me, Hellen you will undergo surgery 2 hours later. You will be transferred to

operation room.”

Patient : “What? Oh my God. 2 hours, Doctor?”(patient so panic)

Doctor : “Yes, Mrs. 2 hours later. Are you ready? ”

Patient : “Hmm yes, Doctor. I’m ready. But….I’m afraid.”

Doctor : “Why?”

Patient : “Because this is first time I will Undergo surgery.  I am so afraid.”

Doctor : “Just relax Hellen. We have to do the best for you. After this, we will prepare to

transferring you to operation room.”

Patient : “Yes, Doctor.”

Doctor : “Ok, Hellen. I will meet a nurse who will prepare everything for it.”

After that, doctor going to nurse station.

Doctor : “Nurse, patient named Hellen in  A3 room. Her condition is good and is ready to be

transfered to the operating room”

Nurse   3: “Yes, Doctor. All the her documentations is ready.”

Doctor and nurse back to patient room to transfer patient to operation room, because patient can’t move alone, she helped move by doctor and nurse.

Nurse   3: “Excuse me mrs, the doctor and I will transferring you from bed to stretcher right now.

Do you willing mrs?”

Patient : “Yes. I will Nurse.”

Doctor : “Just relax Mrs. My partner and I will do the best to do it and safety.”

Nurse   3: “Do you feel comfort with your position now?”

Patient : “Yes, I do nurse.”

Then, nurse help transferring patient from bed to stretcher based on operational standart transferring.

(During in patient room)

Nurse 3: “Ok Hellen, I will help you to move from your bed to strecher. I will hold your arm, and

excuse me Mrs. I need your help to hold her arm. ”

Mother : “Of course, Nurse.”

Nurse   3: “Ok, Mrs.”

After successfully move the patient to the stretcher, patient transfered to room before operating room by her mother and nurse 3. Hellen’s mother  accompanies hellen. In front of operation  room. hellen crying

Patient : “Mommy.” (crying)

Mother : “Its okay. Its okay, dear.”

Patient : “I’m afraid, Mom.”

Mother : “Keep calm, Dear. I always pray  for you.”

Patient : “Nurse may my mother accompany me in this room?”

Nurse 1: “I’m so sorry, your mother can’t enter this room, for the  fluently  procedur of you while undergoing surgery. All of people no entry except medical team.

Mother  : “Please help my child  doctor, nurses. I trust that my child will be okay.”

Nurse 1 : “Thanks for your attention mrs. We will do the best for your mother.”

The nurse handed over the patient with Nurse 2 and Nurse 3 in the transfer room. The room attendant handed over the patient along with a handover report signed by the room attendant and the operating room attendant and written in the registers of the operating room.nurse 1 give medical record, inform consent,and result laboratory test.

Inside the operation transfer room ...

Nurse 1: “Nurse, this patient scheduled surgery this afternoon at 2.00 pm named hellen 25 years from Lotus Room No A3, this completeness of the event news and completeness of medical record, inform consent,and result laboratory test of patients.”

After that, nurses 2 and 3 check the completeness of the official report, the completeness of the identity, the patient's medical record, the patient's general condition, the license of action and other supporting facilities such as medicine and blood supply.

Nurse 2: “What about the patient's own psychic condition?”

Nurse 3: “Is the patient anxious?”

Nurse 1: “Before being brought here, the patient feels fear because this is his first experience of surgery.”

Nurse 2: “Well if that's the kind of patient we're going to deal with immediately.”

Nurse 3: “Well nurse, thank you for taking the patient to the operation room.”

After the handover is done, the patient is taken directly into the temporary room and transferred from the gurney to the bed that is in the temporary room

Doctor : “Hellen can you move by your self?”

Patient : “Can’t doctor, but i can stand up.”

Doctor : “Ok, the nurse will help you to move.”

Nurse 2: “Okay, I will hold your right arm.”

Nurse 3: “I will hold your left arm.”

 (Nurses 2, 3 help patient to move)

After moving the patient from the gurney to the bed in the temporary room, then the nurse asks the patient for permission to change the patient's clothes in standard operating outfits.

Nurse 3: “Sorry, Hellen, I'm going to change your shirt with a surgical outfit. Are you willing?”

Patient : “Yeah nurse, please.”

Nurse 3: (changing clothes to the patient and checking the jewelry used) “Excuse me, will I check if my mother is wearing jewelry?”

Patient : “Yes nurse please.”

Nurse 3: “Well it's over. I will give  jewelry to your family.”

After helping to replace the patient's clothes and checking the jewelry. Then nurse 3 takes the necessary precautions for preoperative.

Nurse 3: “Excuse me,. I will replace the infusion in your hands with a new IV to replace fluids and drugs through intravenous, after which I will check the mother's blood pressure before surgery. Are you willing?”

Patient : “Yes, I will  nurse.”

Nurse 3: “After that, I will put urine catheter, pulse oximeter / ECG Monitor.”

Patient : “Yeah nurse, please.”
